About the Role:

The Northern Adelaide Local Health Network is seeking a Senior Aboriginal and Torres Strait Islander Health Practitioner/Cultural Support Worker to join our Tuberculosis Outreach Service.

The Watto Purrunna Aboriginal Health Service of NALHN delivers primary health care to Aboriginal people through contemporary best practice models of health care.

In this role, you will be pivotal in improving the health and wellbeing of Aboriginal and Torres Strait Islander communities. You will work closely with clients, their families and healthcare professionals to provide culturally appropriate support, education and advocacy.

Key Responsibilities:

  • deliver comprehensive case management services to Aboriginal and Torres Strait Islander clients with tuberculosis or at risk of developing tuberculosis
  • facilitate effective communication and understanding between clients, families and healthcare professionals, ensuring cultural safety and sensitivity
  • conduct home visits, community outreach and follow-up care to support clients' treatment adherence and overall wellbeing
  • collaborate with multidisciplinary teams to develop and implement culturally responsive care plans and health promotion initiatives
  • advocate for the rights and needs of Aboriginal and Torres Strait Islander clients within the healthcare system

To be eligible for this position, you must hold a Certificate IV Aboriginal & Torres Strait Islander Primary Health Care (Practice) or equivalent. Hold registration with the Aboriginal and Torres Strait Islander Health Practice Board of Australia.
