About the Role

Sydney Local Health District has a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.

In this position, you are expected to provide a high quality community based Social Work service to People Living with HIV (PLHIV), carers and their families in order to ensure the provision of optimal health outcomes for the patients/clients of the service.

Ideal Candidate

  • Holds a Bachelor or post graduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers.
  • Knowledge, skills and clinical experience in Social Work clinical practice.
  • Able to work as a member of a multidisciplinary team.
  • Current NSW Driver's Licence (P1 and P2 acceptable).
