The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West, Great South, Upper wheatbelt and Peel Regions of Western Australia. SWAMS delivers a wide range of Services including GP Clinics, Primary Health Care, Counselling, NDIS and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development.
We are looking for a Senior Community Support Worker to join our growing team, based in Mandurah.
The primary purpose of the Senior Community Support Worker is to provide support and mentoring to a team of Community Support Workers providing in-home, direct care to the eligible aged members of the community in their own home.
Responsibilities include (although are not limited to): Providing direct service delivery and ensure CSWs provide quality service in delivering in home care services including domestic assistance, transport, meal preparation, home maintenance, personal care as well as individual and group social support. This role will also include supporting the coordinator in rostering and scheduling in-home care services with available employees, undertake reporting activities together with other relevant documentation,.
Essential Criteria
• Minimum Certificate III Individual Support or similar, or willingness to enrol into qualification by end of probation period (6 months).
• Experience in working with Elders and/or aged and community care services.
• Demonstrated communication and interpersonal skills, including the proven ability to liaise sensitively and effectively with Elders, that provides high quality client service to a diverse range of people in a professional manner while maintaining confidentiality and discretion.
• An understanding of the health and social issues impacting Aboriginal people.
• Demonstrate good time management skills – staff must have the ability to work effectively to meet deadlines and work within timeframes set.
• Ability to complete required paperwork and utilise mobile data reporting tools.
• Ability to work as a member of a team or independently with minimal supervision.
• Experience using applications, Microsoft Word, Excel, databases, email and the internet.
Desirable Criteria
• Aboriginal or Torres Strait Islander descent under section 50(d) Equal Opportunity Act.
• Current knowledge of legislative obligations for Equal Opportunity, Disability Services, Workplace Safety and Health and how these impact on employment and service delivery.
