About the opportunity
The Residential In-Reach Clinical Nurse Consultant (CNC) plays a crucial role in:
- Leading and responding to patient deterioration and clinical issues raised by Residential Aged Care Staff
- Identifying and supporting at-risk patients through initiating proactive assessment and treatment plans
- Facilitating safe hospital transfers/ bridging care for patients returning to Residential Care from Hospital
- Delivering education and clinical support to multidisciplinary teams
- Enhancing escalation pathways in collaboration with medical, nursing and allied health teams within Hospital in the Home (HITH), Geriatric Evaluation and Management (GEM) at Home, Community Palliative Care and the Hospital

To thrive in this role, you will have
- Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve
- Demonstrated proficient clinical skills to assess and manage clinical deterioration within aged care setting
- Understanding of hospital avoidance models of care for Residential Aged Care/ Care Outside the Hospital Walls
- Must have current registration as a Registered Nurse, with the Australian Health Practitioner Regulation Agency and an unencumbered Drivers Licence.
You will also be required to provide evidence of, or in the process of obtaining:
- A current Police Record Check and Working with Children Check.
- Proof of current influenza vaccination and other vaccination status consistent with public health orders.
