Role Description

This is a contract, preferable on-site role in Toowoomba, QLD for a Mental Health Nurse/Cl;inical Care Coordinator. The Mental Health Nurse/Clinical Care Coordinator will be responsible for managing client appointments, coordinating care plans, short-term therapeutic interventions/assessments and triage and connection. The role will include liaising with clients, healthcare providers, and other stakeholders to ensure smooth service delivery. Additionally, the Mental Health Nurse/Care Coordinator will be tasked with maintaining client records, supporting the clinical team, and ensuring compliance with relevant regulations and standards.

Qualifications

  • Registered Nurse with experience in Mental Health
  • AHPRA registered with Mental Heatlh Credentialing or willingness to work towards Credentialing.
  • Excellent organizational, time management, and administrative skills
  • Strong interpersonal and communication skills
  • Experience with client management and care coordination
  • Familiarity with healthcare regulations and standards
  • Ability to work independently and collaboratively in a team environment
  • Proficiency in using office software and client management systems
  • Knowledge and experience with evidence-based mental health clinical interventions
  • Bachelor's degree in Healthcare Administration, Psychology, or a related field is preferred
