We are looking for a compassionate and motivated Case Manager to join our Eastwood office on a full-time basis. In this role, you will work closely with clients receiving Home Care Packages, ensuring they receive quality, person-centred services that help them live independently at home.

What you'll do:

  • Manage a portfolio of Home Care Package (soon to be Support At Home) clients.

  • Develop, review and monitor care plans to meet client goals.

  • Provide advice and guidance to clients and their families.

  • Coordinate the delivery of all services by Support Staff, including scheduling/rostering

  • Monitor package budgets and ensure compliance.

  • Work Health and Safety issues/Risk Management

  • A mixture of being in the office and being out in the community visiting clients

What you'll bring:

  • Qualification in Nursing, Community or Human Services, Case Management, Psychology, Social Work, Allied Health or Rehabilitation Case Management.

  • Experience in case management and aged care.

  • Strong knowledge of HCP funding and Aged Care Quality Standards.

  • Great communication and problem-solving skills.

  • Excellent documentation and computer skills

  • Police Check / NDIS Worker Screening Check (or willing to obtain).

  • Unrestricted current Australian driver licence and reliable car.
