Occupational Therapist

About the Position:

The Occupational Therapist & Clinical Care Partner plays a pivotal role in coordinating and delivering high-quality allied health services within a home care setting. This position is responsible for overseeing the services of Occupational Therapists professionals (Associated providers) and ensuring the effective provision of support-at-home services, including Allied Health interventions and Assistive Technology/Home Modifications (AT-HM). Working collaboratively within the Home & Community Support Team and reporting to the General Manager, this position is responsible for managing a dedicated caseload of clients across a range of Government-funded and private aged care programs, including Home Care Packages and the Support at Home Program.

The Occupational Therapist & Clinical Care Partner provides comprehensive support to clients by conducting home-based assessments, developing individualised care and support plans, equipment and services recommendations and working with the client to implement support for independence and safe mobility in the home and to facilitate timely access to appropriate services.

This role requires a thorough understanding of the Support at Home Program, including working in accordance with the Support at Home Program Manual, aged care legislation, and the Aged Care Quality Standards. Occupational Therapist & Clinical Care Partner must ensure all care is planned and delivered within these frameworks, supporting compliance, safety, and best practice.

Operating within JCS's person-centred, Wellness and Reablement approach, the Occupational Therapist & Clinical Care Partner ensures services are tailored to meet individual goals, promote independence, and support active, healthy ageing. The role requires strong collaboration with other care team members, clinicians, allied health professionals, and external providers to ensure integrated and responsive care.

This position is critical to maintaining high standards of service delivery and upholding JCS's commitment to cultural sensitivity, consumer choice, and continuous improvement.

About you:

The ideal candidate is a registered Occupational Therapist with a strong understanding of aged care frameworks and a commitment to person-centred, culturally sensitive practice. They must demonstrate empathy, effective communication, and teamwork within multidisciplinary settings. Experience in community-based care, client assessments, and support planning is preferred, along with proficiency in Microsoft Office and knowledge of government-funded programs. A current police clearance, driver's licence, and access to a reliable vehicle are required, along with a willingness to learn about Jewish cultural practices.

Key Responsibilities

Coordination and Administration

The Occupational Therapist & Clinical Care Partner conducts assessments, develops personalised care plans, and prescribes assistive technology and home modifications to support client independence and safety. They collaborate with clients, families, and health professionals to deliver evidence-based interventions and contribute to Wellness and Reablement initiatives. The role includes making referrals, maintaining accurate documentation, participating in quality meetings, and ensuring services align with aged care standards. It also involves supporting client choice, promoting staff wellbeing, and contributing to continuous service improvement within a multidisciplinary team.
