**About the role
Community and Oral Health is leading health service delivery in the transitions of care space. Community and Oral Health (COH) has inpatient rehabilitation services, along with Hospital in the Home (inpatient care delivered in the patient's home) as well as a raft of services that deliver face to face services in the patient homes as part of our community service delivery. The transitions of care space also provide a variety of new and exciting opportunities for pharmacist to be at the forefront of patient centred care. About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Deliver evidence based clinical pharmacy services appropriate to the model of care associated with each particular COH community and bedded services. This includes, but not limited to, community palliative care services, transition care programs, post-acute care services and residential transition programs.
As experience builds, apply increasingly independent clinical judgement to more complex clinical decision making under the clinical practice supervision or professional guidance of a more senior clinician
Participate in quality and service improvement activities to continually improve patient care and experience
Provide input in the development of pharmacy clinical practices, procedures and protocols
Participate in continued delivery of pharmacy services within each particular COH community service including but not limited to Community Palliative care, Transition Care Programs, Post Acute Care Services, residential setting and inpatient rehabilitation wards.
