Aboriginal Cancer Care Coordinator

The Cancer Care Coordinator is a regional position responsible for working with the Regional Primary Health Care Services across Maari Ma's region, to provide culturally safe, holistic support for Aboriginal and Torres Strait Islander patients and their families throughout the cancer journey.

The position works as part of a multidisciplinary team, liaising between referring GP and specialists, providing practical support with a focus on patient education to support self-management, care navigation, family support and promotion of recovery. The position works collaboratively with the range of service providers involved in patient care, to ensure culturally appropriate services are provided.

Salary up to $95,268 gross per annum plus super (dependent on qualifications and years of experience)

To be successful you will need:

· Cultural Knowledge and Connection:

o   Strong understanding of the cultural, social, and health-related needs of Aboriginal and Torres Strait Islander communities.

· Experience in Aboriginal Health and Advocacy:

o   Proven experience supporting Aboriginal and Torres Strait Islander communities in navigating the healthcare system.

o   Understanding of cancer care pathways, treatment processes, and survivorship care.

· Strong Interpersonal and Communication Skills:

o   Ability to build strong relationships with patients, families, and healthcare providers.

o   Effective communication skills to advocate for patients and influence culturally safe practices in cancer care settings.

· Community Engagement and Collaboration:

o   Ability to engage and empower Aboriginal and Torres Strait Islander communities in health-related matters.

· Organisational and Coordination Skills:

o   Ability to manage multiple priorities and navigate complex healthcare pathways.

o   Strong problem-solving skills and adaptability to challenges in cancer care coordination.

o   Ability to work both independently and within a dynamic multidisciplinary team

· Demonstrated understanding of and commitment to the principles of primary health care and how they apply to the prevention and management of chronic disease.

· Demonstrated ability to share your skills and knowledge and teach Aboriginal Health Worker Trainees in the workplace.

· Computing skills relevant to the position and competent in the use of information and communication technology including Microsoft Word, email and internet applications and the ability to quickly acquire skills in other applications.
