About the Team
The Child Health Service (CHS) provides universal, multidisciplinary primary community health care services for children from birth to 8 years who reside in the catchment area for the Child and Youth Community Health Service (CYCHS), CHQ. The service seeks to achieve positive early childhood health and development outcomes through surveillance and monitoring of child growth and development and addressing family areas of concern using an early intervention, relationship-based framework, to reduce and minimize the impact of risk factors and strengthen parents/carers' capacity to support their child's ongoing health and development needs.
About the Role
Provide support and governance to the clinical staff via CHS Clinical Nurse Consultants. These positions also provide support to liaison with key stakeholders utilising the Child Health Liaison Model. This includes local maternity providers.
Provide support to the wider CHS service with the provision of data, workflow and recruitment.
Provide leadership, support and clinical practice facilitation to the Grade 5 Registered Nurses undertaking the Transition Support Program-Child Health.
Develop and maintain collaborative relationships with other health professionals, government, non-government and community agencies to enhance continuity of care for children and their parents/carers in accordance with the Child Health Liaison Model.
