About the Role

We're looking for a Clinical Partners clinician & Hospital support program clinician to join our team.

This unique role combines responsibilities across both our Clinical Partners Program (CPC) and Hospital Support Program (HSP), offering a diverse and rewarding scope of work – from patient onboarding through to post-operative recovery.

In this role, you will:

Clinical Partners Program (CPC):

Support patient enrolment and onboarding by reviewing and guiding care plan processes.

Provide reassurance and guidance to patients undergoing total joint replacement, in line with their referring surgeon's instructions.

Work closely with Health Navigation Specialists to deliver our signature "red-carpet" patient experience.

Hospital Support Program (HSP/PHI):

Deliver perioperative nursing care in a short-term post-operative program.

Support patients in their recovery journey and help reduce hospital readmissions within 30 days post-surgery.

Provide ongoing care, education, and reassurance to patients after discharge.

What We're Looking For

Current unrestricted AHPRA Registered Nurse

Minimum 1-2 years of post-graduate nursing experience in an acute or perioperative setting

Strong ability to multitask and manage competing priorities

Excellent clinical judgment and patient-centered communication skills

Tech-savvy and confident using digital platforms and electronic health records

Adaptability to work across two dynamic programs

A collaborative, team-focused approach

Desirable:

  • Clinical experience in perioperative, triage, telehealth, or transitional care settings
  • Strong customer service skills, with a focus on patient-centered care
  • Flexibility to work across two programs as required
  • Work Arrangements

Work Arrangements

Full-time or four days per week (Mon-Fri, 9 AM – 5 PM AEST/AWST).

Virtual role – You must have a reliable home internet connection and a private dedicated workspace.
