The Role
Centacare is seeking a dedicated Clinical Care Partner to join our
Moreton bay - Caboolture
team on Part-Time basis (30 hours per week) and make a meaningful impact on the lives of older Australians. In this role, you will combine nursing expertise with care coordination to ensure that clients receive person-centred, evidence-based, and culturally safe care. You will work closely with clients, their families, Care Partners, Support Workers, Allied Health Professionals, GPs, and specialists to support older Australians in living safely, independently, and with dignity in their own homes.
As part of your responsibilities, you will also contribute to Restorative Care, End-of-Life pathways, and provide leadership in clinical governance and multi-disciplinary team (MDT) activities. This role requires someone with excellent clinical judgment, communication skills, and the ability to work collaboratively within a dynamic team.
Key Duties
- Deliver nursing services (wound care, medication management, dementia support).
- Monitor health, escalate concerns to GPs/specialists, and contribute to clinical governance, incident investigations, and infection control.
- Assist with SIRS reporting and contribute to risk management and quality improvement strategies.
- Build trusting relationships with clients, families, and carers.
- Promote shared decision-making, collaborate with Care Partners, Allied Health Professionals, GPs, and specialists to ensure holistic care, and facilitate case conferencing.
- Maintain accurate, timely documentation and ensure compliance with Aged Care Quality Standards and SIRS obligations.
- Lead multidisciplinary case conferences and ensure optimal outcomes for clients.
- Manage care budgets effectively and liaise with contractors to ensure efficient use of resources.
- Complete comprehensive assessments and develop individual care plans within 7 days of service commencement.
- Ensure care plans reflect client goals, preferences, and risks.
Skills and Experience:
- Current AHPRA registration as a Registered Nurse with at least 3 years of experience.
- In-depth knowledge of aged care practices and community-based clinical care.
- Demonstrated experience in care planning, case management, and clinical decision-making.
- Proven ability to work effectively with multidisciplinary teams, including Allied Health professionals and GPs.
- Proficient in digital care systems and comfortable using technology for case management.
- Strong communication, cultural competency, and empathy when interacting with clients and their families.
- A valid driver's license, Blue Card, NDIS Worker Screening Clearance, First Aid/CPR certification, and an Australian Federal Police check.
Essential requirements for the role:
- Experience in caring for people with dementia, palliative care needs, or complex health conditions.
- Training in trauma-informed care, cultural safety, or restorative practice.
- Familiarity with My Aged Care systems and digital case management tools.
- Previous experience in mentoring or supervising staff.
- Knowledge of SIRS reporting and regulatory compliance.
