What you'll be doing

Key Responsibilities

  • Provide high-quality, person-centred clinical care and advice to clients, carers, families, and other healthcare professionals, ensuring that all therapeutic interventions are evidence-based, culturally appropriate, and delivered within professional, legal, and ethical boundaries, in alignment with the Aged Care Quality Standards and SCHADS Award requirements.
  • Actively participate in staff meetings, case conferences, and inter-professional forums, contributing to the planning, evaluation, and continuous improvement of physiotherapy and allied health services in consultation with the Manager.
  • Establish individualised care plans and functional goals in collaboration with clients and their carers, ensuring that plans are reflective of client needs, preferences, and clinical assessments, and are regularly reviewed for relevance and effectiveness.
  • Maintain accurate and timely documentation of all client interactions, including assessments, progress notes, referrals, care plans, education provided, and multidisciplinary communications, in accordance with organisational policy, privacy legislation, and professional standards.
  • Engage in regular formal supervision and reflective practice, participating in ongoing professional development and performance review processes to ensure clinical competency, compliance with SCHADS Award obligations, and high standards of service delivery.

What we're looking for

  • Tertiary qualifications in Physiotherapy and current registration with AHPRA, with a minimum of three years' clinical experience in aged care and/or home care settings.
  • Proven experience delivering physiotherapy services in both residential aged care and community (home care) environments, including functional assessments, pain management, and mobility interventions.
  • Strong clinical knowledge of geriatric care, including age-related conditions, falls prevention, rehabilitation, chronic disease management, and maintaining functional independence in older adults.
  • Demonstrated ability to conduct comprehensive physical assessments of older adults and individuals with physical disabilities, and to develop, implement, and evaluate person-centred care plans.
  • Excellent communication and interpersonal skills, including the ability to build rapport with clients and families, adapt communication styles for diverse audiences, and provide education in an accessible and respectful manner.
  • Proven ability to work independently in home-based environments, as well as collaboratively within a multidisciplinary team across community and aged care settings.
  • Strong analytical and problem-solving skills, with the ability to prioritise, exercise sound clinical judgement, and develop tailored solutions to meet individual client needs.
  • Demonstrated digital literacy, including competence in using clinical documentation systems, electronic care planning tools, and Microsoft Office applications.
