About the Role

The Social Work team at Dandenong Hospital is seeking an experienced Grade 2 Social Worker to join the General Medicine stream. This is a full-time, ongoing position, based at Dandenong Hospital Social Work Department.

In this role, you will provide high-quality psychosocial assessment, planning and intervention as part of a supportive and collaborative multidisciplinary team. You will carry a designated caseload, applying evidence-informed practice to deliver optimal outcomes for patients and families. This role also offers the opportunity to grow your leadership skills, contribute to projects and research, and take on extended clinical responsibilities.

About Us

The Social Work Service at Monash Health provides psychosocial support and advocacy for patients and families through assessment, counselling, discharge planning, information provision and access to community resources. Social Workers play a pivotal role in responding to social complexity and are valued members of the multidisciplinary team.

The service operates during standard business hours (Monday to Friday), with weekend and on-call services provided at Casey, Clayton, Dandenong and the Victorian Heart Hospital.

About You

You will bring a strong working knowledge of service systems including Family Violence, Child Protection, My Aged Care, the Transition Care Program and the National Disability Insurance Scheme (NDIS). Alongside direct clinical care, you will contribute to quality improvement and research activities, ensuring service delivery aligns with the AASW standards, credentialing, supervision, code of ethics and practice guidelines. Your skills will include:

  • Specialised expertise in complex psychosocial assessment and intervention.
  • The ability to provide a high-quality, evidence-based social work service.
  • Experience collaborating with internal and external stakeholders to deliver patient-centred care.
  • Strong written and verbal communication skills.
  • Commitment to supervision, professional development and continuous improvement.

Qualifications & Requirements:

  • Bachelor of Social Work, Master of Social Work Qualifying, or equivalent (AASW assessed).
  • Eligible for AASW membership (mandatory for overseas-trained applicants).
  • Current Victorian Driver's Licence.
