PRIMARY OBJECTIVES OF POSITION

Direct Support & Case Management


• Provide intensive outreach support to Aboriginal families under the 200-hour response model (including 40-hour stepdown).


• Conduct weekly home visits to support family and Child Protection case goals.


• Deliver culturally safe, trauma-informed interventions to strengthen parenting and child wellbeing.


• Address risks such as family violence, AOD, mental health, and parenting capacity.


• Coordinate care by gathering support networks and facilitating regular Care Team and Professional meetings.

Assessment, Planning & Cultural Safety


• Work with families to develop culturally informed action plans.


• Conduct risk and safety assessments using SAFER and MARAM frameworks.


• Regularly sight children, including infants, and assess their developmental needs.


• Embed cultural identity and connection in all service delivery.

Collaboration & Advocacy


• Work closely with Child Protection, ACCHOs/ACCO's, schools, legal and specialist services.


• Support families to access appropriate services.


• Advocate for families' cultural and wellbeing needs in care teams and court settings.

Administrative Duties


• Maintain accurate, confidential case notes, assessments, and progress reports.


• Input data into reporting systems in a timely manner.


• Participate in supervision, team meetings, and ongoing training.

Program Scope

The FPR Practitioner supports Aboriginal families through:


• Preservation: Preventing child removal by ensuring safety in the home.


• Reunification: Supporting safe, timely return of children to family.

This involves working collaboratively with Child Protection and other services to meet goals, particularly in preparation for Section 18 transition. Work is intensive and holistic, including secondary consultation and support across multiple risk domains.

Family and Community Support

– Engage families with diverse, complex needs and support cultural identity.

– Provide crisis support and practical strategies through role-modelling and coaching.

– Strengthen parenting and developmental outcomes through regular contact and tailored plans.

– Elevate children's voices in planning and care team involvement.

– Maintain confidentiality and uphold the best interest of children.

Risk and Safety

– Conduct risk assessments and safety planning with supervision as needed.

– Use MARAM (brief/intermediate) and SAFER frameworks.

– Liaise with Child Protection for Section 38 and L17 matters.

Organisational & Reporting Responsibilities

– Complete accurate documentation for funding, legal, and compliance purposes.

– Provide supervision to placement students.

– Attend service alliance meetings and professional development.

– Use and regularly update staff engagement plans.
