Officer, Victoria, Australia Caring 4 People Provider pty Ltd Full time $60,000 - $80,000 per year

Duties Includes

  • Psychosocial disability (mental health) Supports
  • Plan review reports
  • Biopsychosocial and family assessments
  • Individual planning/ Budget Planning
  • Implementing plans to achieve individual goals
  • Allied Health Team collaboration work
  • Coordinate the delivery of support and services
  • Supports for individuals, families and those experiencing social disadvantage
  • Providing Coordination of supports or Specialised Support coordination

Key Requirements

  • A bachelor's degree in a relevant discipline in Social Work
  • AASW Registration
  • Demonstrated well-developed oral, written and interpersonal communication skills;
  • Demonstrated ability to work well independently and cooperatively as a member of a team;
  • Established ability to listen actively and demonstrate empathy;
  • Well-developed organisational skills, the ability to manage time effectively and prioritise tasks;
  • Working with Children card; NDIS Worker Screening; Police check (within 6 months); current Drivers Licence, car insurance and roadworthy vehicle.
  • Computer literacy – efficiency in Microsoft Office and Excel;

Desirable

  • Experience working with clients with dual diagnoses. Experience in Forensic Disability or Forensic Mental Health is highly desirable;
  • Demonstrated knowledge of NDIS and experience working with NDIS Participants;
  • A willingness and ability to travel by car,
  • Experience working with Culturally and Linguistically Diverse (CALD) people and their support networks.
