Sunraysia Community Health Service (SCHS) is a dynamic organisation promoting health, wellbeing and independence in a social model of health. The organisation is a source of primary and preventive care for our community members who are at risk of poor health outcomes, and experience acute health disparities. SCHS understands the influence of the Social Determinants of Health upon health inequities experienced by community members and work with consumers and community members, to structure their care around the context in which they live and work.

The Program

The Program Support Officer will support referrals and service access to the SCHS programs, assisting the community to navigate the health and welfare system both internally and externally to the organisation. Program Support will facilitate streamlined and timely access for clients to the services they require and provide a range of administrative and general support to the Managers and staff.

The position will also include tasks such as:

  • Manage referrals into the organisation with a focus on ensuring SCHS interactions with clients are streamlined, simple to navigate, transparent and consistent.
  • Contacting clients to arrange appointments as well as collecting required information and inputting into an electronic database.
  • Assist the program managers with monitoring and processing of referrals, staff's diary management, monitoring and reporting on service delivery and program related administrative tasks as required.
  • Assist teams with the purchasing of consumable products and completing the financial recording

What you'll bring:

  • Highly developed interpersonal and communication skills and ability to work within a team
  • Ability to collect information in a sensitive manner, with practical regard to cultural requirements, language issues, special communication needs, privacy and confidentiality
  • Well-developed computer skills including word processing, database management and electronic information exchange
  • Ability to be reliable and to work with minimal supervision
