About the role

The Nurse Coordinator will support our veteran community enrolled in the Coordinated Veteran's Care program to achieve their health goals, optimise their health outcomes and avoid unplanned hospitalisations. Additionally the role may include annual in home health assessments for patients as directed by the patient's usual GP and, where appropriate, the preparation of GP Management Plan and Team Care Arrangements.

Key responsibilities of the role include:

  • Continuing the care of our patients already enrolled in the CVC program in close collaboration with the patient's usual GP.
  • Undertaking annual comprehensive home health assessments to identify new or emerging health and care needs
  • Assisting in the preparation and review of care plans in consultation with the patient and GP
  • Liaising with all members of the health care team to ensure services are well coordinated, including scheduling appointments where required
  • Maintaining regular contact with the participant in line with the CVC Program Guidelines
  • Supporting patients to better self-manage their chronic conditions and maintain healthier lifestyles
  • Identifying potentially eligible patients to participate in the CVC Program
  • Supporting GPs and the practice team to bill appropriately and within the relevant MBS/ DVA guidelines

Skills & experience

  • Registered Nurse or Enrolled Nurse with at least 2 years' post registration experience preferably in chronic disease management, general practice and/or a care coordination role
  • Excellent oral and written communication skills, including telehealth and face to face
  • Highly developed interpersonal skills with strong ability to develop collaborative relationships with patients, their carers, doctors and the broader care team.
  • Working knowledge of key chronic disease areas (eg, diabetes, CVD, respiratory medicine, renal disease, mental health and related risk factors)
  • Current and unrestricted driving license and access to fully insured vehicle
  • Exposure to General Practice software tools and relevant MBS items (desirable)
  • Experience working in the veteran community and/or understanding of their specific health needs (desirable)
