Being a Youth Transition Officer offers you the opportunity to work closely with the Moreton Bay CLO, other government agencies and stakeholders to develop tailored advice and support to young people and their families and carers.

About The Role
This role is based in the Metropolitan Regional Offices (North Lakes and Nundah) and the Children's Courts across Moreton Bay. As the Youth Transition Officer, you will build strong relationships with young people and families who may be involved in, or at risk of being involved in, the Youth Justice System.

The Youth Transition Officer works closely with Court Liaison Officers and other regional staff involved in student engagement and re-engagement. Key to the role will be your ability to develop a strong working knowledge of relevant legislation, policy, and procedures across education and youth justice contexts, including the operation of the Children's Court and multi-agency collaborative forums. This knowledge, alongside strong connections to alternative educational providers, experience in working collaboratively with state school leadership teams, and an enhanced ability to assess young people's educational and support needs, will enable you to scaffold their reintegration into an appropriate education pathway.

Evidenced experience in working with families that are Aboriginal and/or Torres Strait Islander and, more broadly, culturally diverse, will be important in your daily work.

Responsibilities Include
As the Youth Transition Officer, you will:

• Liaise with a range of internal and external stakeholders, including alternative education providers, training organisations, and community organisations, providing a departmental perspective.
• Refer young people, including those requiring minimal intervention and those with highly complex educational needs, to avenues for direct support/advocacy.
• Develop an education plan to support a young person's transition into a suitable educational pathway in consultation with the young person, their family, Child Safety, Youth Justice, and other relevant stakeholders.
• Ensure privacy and confidentiality requirements are adhered to, ensuring that information obtained from young people, families, and departmental information systems is used within legislative and policy requirements.
• Establish and maintain relationships with internal and external stakeholders to assist with young people's re-engagement in educational pathways.

• Previous experience working with young people and families.
• A localised understanding of the demographics specific to Moreton Bay.
• Experience in liaising with educational providers.
• Best practice case management skills.
• Confident communication skills.
