Care Coordinator. Permanent Full Time

About the Role

As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Newcastle and the Hunter Region. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.

Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems. You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering growth among our aged care clients.

This is a new position arising from the launch of our services in this region. The successful candidate will be driven to support growth and establish a strong reputation for LWB among our clients and the local community. This position is based in Newcastle with some travel required to clients' homes across the region as needed. This is an exciting opportunity for the ideal candidate to take ownership of the role and accelerate their career growth.

Key Responsibilities

• Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders
• Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits
• Monitoring the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clients
• Collaborate effectively with internal stakeholders to ensure clients' needs are met including our Rostering team and Practice Leads
• Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
• Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community

Skills & Experience

• Demonstrated Aged Care or other clinical case management experience
• Understanding of Aged Care funding and service delivery
• Confidence in utilising a variety of technology including mobile devices and computers
• Proficiency with Microsoft Office, Outlook and database systems
• Ability to effectively build relationships with diverse stakeholders
• Strong communication skills and confidence in delivering training
• Ability to multi-task and work autonomously in a fast paced environment
• A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel
