Our team is expanding, and we are looking for a motivated and capable Care Coordinator/Hr to help us strengthen service delivery and grow our workforce.
Position Overview
As a Care Coordinator, you will play a pivotal role in overseeing participant care, coordinating services, and recruiting quality support staff. You will work closely with participants, families, stakeholders, and the wider team to ensure seamless support, with responsibilities across client management, scheduling, relationship building, and HR support. This role also involves using apps like Sameview and ShiftCare to enhance transparency, scheduling, and communication.
Key Responsibilities
1. Participant Coordination & Care Planning
- Conduct intake assessments, support needs evaluations, and risk assessments.
- Develop and monitor personalised care plans aligned with NDIS goals.
- Oversee participant progress and adjust supports as needed.
- Maintain comprehensive documentation in accordance with NDIS standards.
2. Stakeholder Engagement
- Liaise with hospitals, support coordinators, allied health teams, and families.
- Build and maintain positive relationships to facilitate smooth service transitions.
- Represent Mecura professionally to expand business partnerships.
3. Scheduling and System Use
- Use ShiftCare for scheduling, shift notes, incident reporting, and tracking compliance.
- Use Sameview to manage and share participant goals, progress, and support needs collaboratively with families and professionals.
- Benefits of Sameview: Promotes transparency, enhances collaboration, and aligns care delivery with the participant's goals.
- Ensure participants are appropriately matched with available support staff.
4. Human Resources & Recruitment Support
- Lead recruitment and onboarding of support workers to meet service demand.
- Collaborate with the HR team to complete worker documentation, inductions, and compliance checks.
- Monitor staff performance and provide feedback and support as required.
- Support rostering, conflict resolution, and shift backfilling.
5. Administrative & Reporting Duties
- Keep accurate and up-to-date records on participant files.
- Prepare reports and assist with internal quality reviews.
- Respond promptly to internal and external queries.
Essential Skills & Experience
- Background in business, human services, community services, or HR.
- Minimum 2 years' experience in a client-facing, coordination, or people management role.
- Strong organisational skills with attention to detail.
- Excellent communication and interpersonal abilities.
- Willingness to work across multiple systems and learn NDIS-related software tools.
- Ability to adapt and thrive in a dynamic environment.
Desirable
- Experience in disability, aged care, or health sectors.
- Knowledge of NDIS service delivery frameworks.
- Familiarity with ShiftCare or Sameview (preferred, not required).
Compliance Requirements
- Current Driver's Licence and reliable transport.
- NDIS Worker Screening Check.
- National Police Check.
- Working with Children Check (Blue Card).
- First Aid & CPR Certificate (or willingness to obtain).

Pay: $72,000.00 – $82,000.00 per year
