About the Transition and Restorative Care Programs
The Transition and Restorative Care Programs sit within Aged Care Community Services as part of the Continuing Care Division at Austin Health. The programs are committed to providing high quality safe care for patients while helping them achieve their goals. The programs are provided in both residential and community settings.
About the role
The Grade 2 Physiotherapist will work as a member of the Aged Care Community Services Transition Care and Restorative Care Teams, providing a Physiotherapy service to clients in both our residential and community programs.
The role will see you undertaking assessment and treatment of clients to optimise or maintain functional capacity, and you will have a key role, along with other team members in supporting discharge planning.
A regular day will see you:
- Providing high quality physiotherapy assessment and treatment to clients in both residential and community settings
- Actively participating in case conferences and family meetings as required
- Working closely with the wider TCP team to ensure the delivery of safe and effective client care and supporting d/c planning
About you
You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You'll bring energy and drive to your work and have a keen interest in developing your skillset.
You'll further have:
- A physiotherapy qualification and current AHPRA registration
- Strong clinical skills and demonstrated experience in an aged care or community settings
- Excellent interpersonal and communication skills that support patient care, team engagement and collaboration
- A willingness to think outside the square and think creatively to achieve the best possible client outcomes
