Primary objective

Support the team to achieve the best outcomes for clients by coordinating assessment services and resources in accordance with the Australian

Government Aged Care Legislation and Departmental Polices, including conducting clinical assessments and case coordination for clients and

families eligible for the Aged Care Assessment Program.

Context statement

The Aged Care Assessment Service is a Commonwealth funded program which has legislated delegation authority to approve clients for Residential

Aged Care, Respite Care, Support at Home and Transition Care. The ACAS Team Leader is responsible for compliance of the Australian

Government Aged Care legislation and reforms, ensuring systems and resourcing to meet the KPI's within the multidisciplinary team. ACAS Team

Leader also has a role as an ACAS assessor, delegate, and the management of the triage process through the My Aged Care portal.

Key duties and responsibilities

1. Coordinate Aged Care Assessment Service (ACAS) service delivery in accordance with legislation, policy, and procedures; this includes the

management of financial and human resources in line with Departmental policies and delegations.

2. Provide leadership, supervision, and support for ACAS in relation to collaboration with internal/external stakeholders to ensure the best

possible outcome for clients, including overseeing service provision to clients in crisis situations.

  1. Provide specialist advice on Aged Care issues to Management in relation to planning, implementation, delivery, and evaluation of services.

  2. Assess and coordinate services for ACAS clients and ensure best practice of ACAS staff.

  3. Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken to ensure high

quality, safe services, and workplaces.

  1. Foster a positive workplace culture through exemplary leadership practices and role modelling behaviour.

Selection criteria

Essential

1. Professional qualification and registration (where applicable) in a related health field, i.e. Allied Health Professional or Registered Nurse.

2. Demonstrated significant experience working with aged clients, including the implementation of culturally appropriate holistic assessments

and interventions for clients to enable them to live in their preferred environment.

3. Demonstrated ability to coordinate a multidisciplinary team, with a proven capacity to effectively apply conflict resolution, negotiation skills

and problem solving and stress management skills.

  1. Demonstrated ability to analyse and manage complex client issues with flexibility, initiative, sound judgement and decision-making skills.
  2. Demonstrated ability to act as a consultant to a wide range of stakeholders and demonstrate a working knowledge of the relevant

Commonwealth and Northern Territory (NT) principles and the legislation underpinning the Aged Care Assessment Service (ACAS).

6. Demonstrated high standard of written and verbal communication skills including the ability to deal with highly sensitive information and

issues with tact and discretion, with special consideration for people from diverse cultures.

  1. A Northern Territory driver's licence or ability to obtain, and ability to travel within the territory for up to 5 days at a time.

Desirable

1. Experience working within the ACAS.

2. Postgraduate qualification in Aged Care and/or Accredited Management qualification.
