Aged Care Connector

Mungabareena Aboriginal Corporation is seeking a dedicated Aged Care Connector (ACC) who will work under the direction of the Community Health Team Leader to connect the local Elders, older Aboriginal and/or Torres Strait Islander people and their families to raise awareness of the aged care entitlements. The role will play a crucial role in improving access to culturally appropriate aged care services for Aboriginal and/or Torres Strait Islander elders and supporting them in maintaining their health, dignity, and independence as they age.

The ACC overarching function is to ensure that support for clients is organised, sequences and supported to enable effective access to appropriate aged care services both internal and external to Mungabareena Aboriginal Corporation.

The ACSC role helps determine the level of local demand for aged care services, identifying existing aged care services, any service gaps in the area and continue advocacy work with clients as required.

In this key role, you will:


• Active community outreach to promote aged care services, identify and engage with potential clients.


• Support clients to identify goals and needs to ensure they get the appropriate level of care and services, which can include referrals to other specialised services and support.


• Serve as a cultural liaison between Indigenous elders and aged care service providers. Bridge cultural gaps, facilitate communication, and ensure that services are culturally appropriate and respectful.


• Support clients' aged care needs through internal referral processes.


• Assist clients with advocacy and support throughout the My Aged Care registration and review processes.


• Provides support to enable client access to appointments.


• Assist with navigating complex administrative processes, resolving issues, and ensuring continuity of care.


• Liaises with the aged care team to continuously review and enhance the role.


• Assist elders to access various aged care services, including medical care, home support services, transportation, and social activities.


• Ensure client care and cultural needs are being met and address any issues with service providers through appropriate internal MAC representatives.


• Building knowledge of local services and organisations clients can utilize.


• Monitor the quality of aged care services provided to Indigenous elders, soliciting feedback from clients and families to identify areas for improvement. Advocate for culturally competent and respectful care practices.


• Maintain accurate records of client interactions, referrals, and outcomes. Prepare reports on service utilisation, gaps in service provision, and emerging needs within Indigenous communities.

This position requires willingness to provide, complete and submit evidence prior to commencement of employment of:

  • A qualification in Heatlh or a related field is desirable
  • A current Driver's Licence.
  • A current Victorian Working with Children Check.
  • A current First Aid & CPR Certificate
  • A current Police Check.
  • A Pre-employment Health Declaration.
