Northern Sydney Families need an experienced Child and Family Health Nurse with experience in the Sustained Home Visiting Program Sustaining NSW Families. The role also requires some management and or leadership experience to help them effectively lead a multi-disciplinary team deliver better outcomes for families in some of our Local Government Areas.

What you'll be doing
The Clinical Coordinator (CC) is responsible for the operational management and leadership of the Sustaining NSW Families (SNF) nurse led home visiting program offered to vulnerable families from pregnancy until their child is two years old.
The Clinical Coordinator is a Child and Family Health Nurse whose clinical role includes managing new referrals to the SNF Service and intake, undertaking initial client (family) engagement and clinical assessments and providing clinical consultation for team members, leading case reviews and may relieve clinical staff during short periods of leave.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Registered Nurse-Division 1 with Australian Health Practitioner Regulation Agency (APHRA) and relevant management tertiary qualification or equivalent work experience, or a combination of study and work experience; and demonstrated commitment to professional development.
- Extensive post registration experience in Child & Family Health Nursing (CFHN);
- Experience working with vulnerable families and a demonstrated understanding of early intervention, including the theoretical basis that underpins sustained home visiting, prevention, and community development.
- Experience in managing a team and knowledge in strategic and operational service planning and quality improvement processes.
