Key responsibilities include:
- Deliver safe, effective, and patient-centred clinical care, ensuring medication safety and optimal medication management
- Collaborate within a multidisciplinary team and the pharmacy department to provide high-quality clinical pharmacy services across the organisation
- Actively participate in ongoing education and training
- Participate and lead quality improvement activities and projects
- Actively participate in mandatory meetings as required and requested
- Maintain core clinical competence specific to the area of clinical practice
- Participate in a weekend and on call roster
Qualifications/experience required:
- Current AHPRA Registration as a Pharmacist
- Completion of post graduate certificate (minimum) in clinical pharmacy, pharmacy practice and other relevant credentialing
- Excellent written and verbal communication skills
Desirable:
- Prior experience in supervision of intern pharmacists, pharmacy students and pharmacy technicians
- Partnered Pharmacist Medication Charting, credentialed or desire to obtain credentialing
- Prior experience in delivery of education to health professionals and patients
