At Transform NDIS Support, we are seeking a dedicated Program Support Coordinator to lead and manage our frontline teams, ensuring high-quality and person-centered support services for our participants.

About the Role
The role reports to the Care Manager and is responsible for overseeing the day-to-day operations of our disability support services. You will lead a team to coordinate services, drive continuous improvement, and build strong relationships with clients, families, and stakeholders, supporting TNS mission to enhance the lives of people with disabilities.
Key responsibilities
- Manage and monitor disability support services to ensure alignment with client needs, NDIS requirements, and budget constraints, including rostering and care plan development.
- Supervise, train, and support team leaders and support workers to uphold professional standards and foster ongoing staff development.
- Maintain strong, person-centred relationships with clients, families, and support networks, while addressing concerns and feedback promptly.
- Collaborate with internal teams and external stakeholders to address emerging risks and ensure safe, responsive service environments.
- Ensure adherence to NDIS standards and legislation, manage incidents and risks, and maintain accurate and up to date documentation and records for each client.
- Drive service enhancements, contribute to performance reporting, and support planning and development efforts alongside the Operations/Care Manager.
Skills, Qualification and Experience
- Certificate III/ Certificate IV/Diploma in Disability and/or Mental Health or related field.
- Must have at least 2 years experience on similar role.
- Current First Aid & CPR (or willingness to obtain).
- Current and valid Driver's License & Registered Vehicle.
- Current Working with Children Check.
- Current NDIS Worker Screening Check.
- MAPA training not mandatory but desirable.

- Demonstrated understanding of a person-centred approach and NDIS standards.
- Excellent verbal and written communication skills and the ability to connect with internal and external stakeholders.
- Ability to follow individual support care plans.
- Ability to develop risk & crisis management plans.
- Ability to lead and mentor team members.
