About the role:

You will be responsible for delivering in-home and community based aged care services that focus on re-enablement, wellness, and increased independence for consumers. Services will align with each individual's Care Plan, developed in consultation with families and key stakeholders.

Key Responsiblities:

  • Monitor and report urgent changes in a consumer's health or environment to the Manager Community Services.
  • Maintain accurate and timely documentation, including:
  • ​Daily journal of visits, tasks and observations
  • Weekly reports and check-ins with the Manager.
  • Encourage consumer and family feedback to continually improve care quality.
  • Uphold strict confidentiality and dignity of all consumers.
  • Work collaboratively with Indigenous Knowledge Centres (IKCs) and the Healthy Lifestyle Program to support participation in social and community activities.

  • A genuine passion for working with Elders and supporting community wellbeing.
  • Strong communication and interpersonal skills.
  • Cultural awareness and a respectful understanding of Ailan Kastom and community protocols.
  • Ability to work independently and maintain professional boundaries.
  • Basic administrative and reporting skills.
  • Minimum one (1) year experience in a similar role.
  • National Police Check upon commencement.
  • Working with Children (Blue Card) for paid employment.
  • Certificate III in Aged Care or related discipline or wiillingness to undertake studies.
  • Current First Aid and CPR (or willingness to obtain).
  • Queensland Driver's Licence.
  • Demonstrated understanding of the issues impacting Torres Strait Islander people together with the ability to communicate sensitively and effectively.
