Based in Toowoomba and working across the beautiful Darling Downs region, you'll join a supportive multidisciplinary team within the Transition Care Program—helping clients regain independence and thrive after hospital care. What your day will look like….

You'll work closely with clients to assess their needs and deliver tailored occupational therapy that supports recovery and independence. Collaboration with a multidisciplinary team will be central to your role, as you contribute your expertise to holistic care planning and service improvements. Whether mentoring students, refining clinical protocols, or travelling across the Darling Downs to extend care, each day brings variety, purpose, and connection.
