We do things a bit differently to other 'care' organisations out there. We don't use agency staff. We build a team of nurses and support workers around our clients to ensure that they have consistency of care, and our employees have regular shifts with the same clients, so they build that relationship. As a Case Manager you play a vital role in the management of our clients and their supports.
We are looking for two additional Case Managers to join our team, and one experienced Support Coordinator. The position's are full time and based in our Head Office in Southport. Ideally you will have had some Case Management experience. In terms of qualifications, we would be interested to hear from you if you are either a Registered or Enrolled Nurse or you could be an experienced Case Manager that has a Degree, Diploma or Advanced Diploma in Nursing/Community Services/Human Services/Case Management/Psychology or Social Work. If you are an experienced Support Coordinator this position could also be for you No Billable hours
Customer service will be important to you, as its important to us You will have strong relationship management skills and be adept at advocating for your clients whilst liaising with multidisciplinary teams. You will be allocated a case load of clients to look after and ensure that the staff for that client are trained and the allied health team is engaged. You will be working with your client towards their goals and will attend multi-disciplinary meetings.
The role is a mixture of being in the office and being out in the community visiting clients, training your support team and onboarding new clients. There are company vehicles available for use. You will be responsible for ensuring that your client rosters are correct prior to roster rollover and we are confident that we can teach you how this is done You will have an eye for detail and the ability to think critically with a focus on finding solutions

To be successful you will have:· A degree, advanced diploma or diploma in Nursing, Community or Human Services, Case Management, psychology, Social Work, Allied Health or Rehabilitation Case Management
· Previous case management and/or supervisory experience
· Support Coordinator experience would be highly desirable
· Ability to work collaboratively in our multidisciplinary team
· Highly developed verbal and written communication skills
· Confidence in managing your time effectively including the ability to work autonomously and part of a team
· IT savvy
· Blue card and NDIS Workers Screening Clearance or willingness to obtain
· Unrestricted current Australian drivers licence
· Be available to participate in the On Call roster, once every 8-10 weeks (and will be paid a generous on call allowance). Don't worry you don't go on the On Call roster until you are fully settled into your role and are deemed confident
