Nepean Hospital is a teaching hospital that provides tertiary referral services in emergency care, coronary care, diagnostics, gynaecology, paediatric, neonatal intensive care/ICU, maternity, mental health, rehabilitation, and surgery. We are seeking an experienced Social Worker to deliver high-quality psychosocial assessment, intervention, and management for patients and families within the Rehabilitation Unit at Nepean Hospital, with support to medical and surgical wards as needed. This role involves complex clinical work requiring advanced reasoning, autonomous practice, and sound professional judgement. You will play a vital role in supporting patients through the emotional and social challenges of illness and hospitalisation, while also contributing to smooth transitions within the hospital system. As part of a multidisciplinary team, you will provide crisis intervention, counselling, advocacy, case management, and discharge planning.

What you will bring to the role

  • A Social Work Degree accredited by the Australian Association of Social Workers
  • Demonstrated experience working in an acute hospital setting and extensive knowledge of the social issues relating to patients undertaking rehabilitation
  • Proven experience in crisis intervention, grief counselling, conflict resolution, supportive and individual counselling. Strong skills in mediation and conflict management, particularly in relation to family conflict.
  • Proven ability in applying sound professional knowledge and clinical judgement when performing novel, complex or critical tasks. Complex cases/ critical tasks will involve conditions and elements that must be first identified and then analysed to discern interrelationships before practice decisions can be made.
  • Demonstrated capacity to provide clinical supervision
  • Demonstrated ability to work independently with minimal supervision and to make clinical decisions within a short timeframe
  • Demonstrated ability to work as a member of a team; basic computer skills; proven time management skills; excellent oral and written communication skills
