Based at Macquarie Hospital but working across the Northern Sydney LHD
What you'll be doing
Joining the Northern Sydney Local Health District (NSLHD) ACAS will mean you will be part of a dynamic team with various sites providing Aged Care Assessment to NSLHD. Excellent education and clinical supervision is provided on an ongoing basis. The team will welcome applications from all those with strong interest and passion for working with older persons and who want to be part of an experienced friendly team of Occupational Therapists (OTs), Physiotherapists (PTs), Social Workers (SWs) and Registered Nurses (RNs). Applicants need to have a strong commitment to working to a high standard and within very tight time frames.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Relevant tertiary qualifications as per clinical discipline (nursing, occupational therapy, social work, physiotherapy), with full AHPRA registration or eligibility for membership of the relevant Australian peak professional body for the relevant discipline.
- Proven high standard of written and verbal communication skills. Written skills need to be precise, clinically sound and completed in a timely manner. Proven ability to use Microsoft Office Programs, electronic patient record systems and e-Learning sites.
- Demonstrated discipline in and clinical experience working in relevant aged care settings and a proven ability to undertake comprehensive assessments, formulate, and implement appropriate care plans for older people.
- Have detailed knowledge of My Aged Care, related aged care programs and services with proven ability to use this knowledge to assess and support both routine and complex aged care presentations.
