About Us
Communify Queensland is a dynamic community organisation supporting people across all walks of life to enhance wellbeing and stability. We deliver a broad range of programs including housing and homelessness services, mental health support, aged care, family and individual support, and community development.
Our Sustaining Tenancies Program helps people who are at risk of losing their housing to stay safely housed. Funded by Brisbane City Council and Queensland Health, the program works in partnership with BRIC Housing, Brisbane Housing Company, and the Department of Housing to provide practical, personalised, and trauma-informed support.
About the Role
We are looking for a compassionate and proactive Brief Intervention & Intake Worker to join our Sustaining Tenancies team. This part-time role (until 30 June 2026, with high potential for ongoing extension) focuses on early intervention and assessment, supporting individuals and families to maintain their tenancies and prevent homelessness.
You will play a vital role as the first point of contact for clients, providing high-quality intake, assessment, and brief case management. Using a strengths-based and trauma-informed approach, you will connect people to the right services, advocate on their behalf, and help them build pathways toward housing stability.
Key Responsibilities
- Conduct client intake, screening, and assessment using motivational interviewing techniques.
- Provide short-term, goal-focused support to individuals experiencing housing instability.
- Develop and implement brief case plans to help clients sustain their tenancies.
- Link clients to appropriate community, housing, health, and social support services.
- Provide outreach support to engage clients in their homes and communities.
- Maintain accurate case notes and reporting requirements in line with program standards.
- Collaborate with internal teams, housing providers, and partner agencies.
About You
You are an empathetic and organised professional with strong casework and assessment skills. You're motivated by making a difference in people's lives and have the ability to work collaboratively in a fast-paced, client-focused environment.
You'll bring:
- Qualifications in social work, human services, or a related field.
- Experience working with people facing complex housing, mental health, or AOD issues.
- Knowledge of housing and homelessness support networks.
- Strong communication and interpersonal skills.
- Ability to work autonomously and manage competing priorities.
- Current driver's licence, National Police Check, and Working with Children Check.
Why Join Communify
- Be part of a values-driven, inclusive organisation making a tangible community impact.
- Supportive, collaborative, and professional team culture.
- Access to training, supervision, and professional development opportunities.
- Salary packaging benefits available.
