We are seeking a dedicated Diabetes Educator to join our multidisciplinary team. This role offers opportunity to provide high quality care and education for client affected by or at risk of diabetes at Winnunga Nimmityjah Aboriginal Health Service.
This exceptionally professional and committed staff member will be responsible for the following tasks:
• Provide high quality care and education to individuals and client groups at risk of or affected by diabetes encouraging optimal self management using evidence based practices
• Assess, plan, conduct and evaluate individual and/or group education and ambulatory care programs for clients at risk of or diagnosed with diabetes and/or their carers
• Provide support and counselling to carers and families within an advocacy role
• Monitor clinical outcomes, measures and the effectiveness of education and support provided to clients, carers and family members
• Use comprehensive specialised knowledge of diabetes to provide a consultancy role and professional development opportunities to other health professionals
• Work within professional standards, current policies and procedures of the service setting, and provide reports and statistics as required and participate in team activities
• Initiate, participate, utilise and disseminate quality improvement and research, which contributes to improving diabetes care
• Evaluate self – practice regularly and participate in the performance appraisal process
• Maintain infection control principles at all times
• Maintain highest standards of professional conduct
We are looking for exceptionally empathetic and resilient person who has:
• AHPRA registration and relevant qualifications as a Diabetes Educator
• deep understanding of Aboriginal and Torres Strait Islander peoples, communities and cultures to ensure culturally safe interactions
• understanding of diabetes pathophysiology, types, and management principles and pharmacological and non-pharmacological interventions, including insulin therapy, oral medications, and lifestyle modifications
• competence to appropriately manage the sensitive information of our clients in line with privacy and confidentiality legislation
• awareness of chronic disease management frameworks and multidisciplinary care models
• strong health education and coaching skills to support self-management
• ability to work effectively within a multidisciplinary team, including GPs, nurses, dietitians, podiatrists, and allied health professionals
• ability to manage competing priorities effectively with great attention to detail and accuracy
• deep care for integrity and professionalism in every interaction (internal and external)
