About the position...

Your LINC (Lived Identity Navigate Connect) is a multi-agency, two-tier mental health outreach service delivered in partnerships with KWY Aboriginal Corporation and Centacare.

Our service offers holistic, flexible and culturally responsive psychosocial support for individuals experiencing mental health challenges and distress across Metropolitan Adelaide and some peri urban post codes.

KWY provides non-acute psychosocial support through the provision of case management to support people to build independence connect with community and improve quality of life over a period of 4 – 12 months. 12 months contract.

Key Responsibilities...

  • Provide complex case management responses for individual clients, within a Social and Emotional Wellbeing Framework.
  • Provide one-on-one, trauma informed mental health support to Aboriginal and Torres Strait Islander individuals by addressing daily challenges and strengthening cultural and community connections.
  • Undertake risk assessments and safety planning including the development and implementation of quality case plans and goal setting.
  • Be proactive in advocating on behalf of individuals and liaise regularly with other service providers for a collaborative approach in supporting clients.
  • Build relationships and work in collaboration with other stakeholders.
  • Work effectively within a team, maintaining team boundaries and confidentiality in a professional manner.
  • Be aware of Aboriginal cultural practices and/or differences and seek cultural consultation to promote inclusive practices.

About you...

The successful applicant will possess skills, experience, and knowledge to support Aboriginal people with non-acute psychosocial challenges and distress. The applicant will operate within a therapeutic, trauma-informed framework. The applicant would have demonstrated knowledge of case management and working with Aboriginal families.

Job requirements…

  • Relevant qualifications and/or experience commensurate with the position in Mental Health, Social Work, Social Sciences and/or Psychology.
  • You must hold a current South Australian Driver's License or a license which permits you to drive in South Australia.
  • Successful candidates will be required to clear and maintain probity checks including National Criminal History Record Check (Police Clearance), Working with Vulnerable People Check and Working with Children Check (applicants need to hold a current WWCC to apply).
