Role Overview:

We are seeking an experienced NDIS Support Coordinator to join our team in Sydney area. In this role, you will assist NDIS participants to implement their plans, connect with service providers, and achieve their goals. You will provide ongoing support to ensure services are delivered effectively and meet the individual needs of each participant.

Key Responsibilities

Participant Support & Planning

  • Work closely with NDIS participants to understand their individual goals, needs, and preferences.
  • Help participants implement their NDIS plans effectively.
  • Develop action steps and strategies to achieve short- and long-term goals.

Service Coordination

  • Connect participants with suitable service providers (e.g., therapy, community, or in-home support).
  • Build and maintain strong relationships with providers to ensure quality service delivery.
  • Monitor services to make sure they align with participant goals and budgets.

Capacity Building

  • Support participants to strengthen their ability to manage their own supports.
  • Educate them on NDIS processes, budgeting, and choice of providers.
  • Promote independence and empowerment wherever possible.

Monitoring & Reporting

  • Regularly review participants' NDIS plans and progress.
  • Document outcomes, challenges, and adjustments needed.
  • Provide feedback and reports to the NDIA (National Disability Insurance Agency) as required.

Problem Solving & Advocacy

  • Address barriers participants face in accessing services.
  • Advocate for participant rights and ensure fair access to resources.
  • Escalate complex issues to the NDIA or relevant stakeholders when necessary.

Compliance & Administration

  • Ensure all support coordination activities meet NDIS guidelines.
  • Maintain accurate participant records and case notes.
  • Follow organisational policies, confidentiality, and safeguarding procedures.

Qualifications and Experience

Education

  • Minimum: Certificate IV in Disability, Community Services, Mental Health, or equivalent.
  • Preferred: Diploma or Bachelor's degree in Social Work, Psychology, Nursing, Occupational Therapy, or other allied health/community services fields.

NDIS Knowledge

  • Strong understanding of the NDIS framework, funding categories, and guidelines.
  • Experience interpreting NDIS plans and assisting participants to utilise their budgets effectively.

Professional Experience

  • At least 1 year of experience in disability services, community support, case management, or related roles.
  • Proven ability to coordinate supports, manage caseloads, and liaise with multiple stakeholders.

Skills & Competencies

  • Excellent communication and interpersonal skills for working with participants, families, and providers.
  • Strong problem-solving and advocacy skills.
  • Organisational and time-management skills, with the ability to handle competing priorities.
  • Ability to maintain accurate records, prepare reports, and ensure compliance with NDIS standards.

Other Requirements

  • Current Working With Children Check (WWCC) and National Police Check.
  • First Aid/CPR certification (preferred).
  • Current driver's licence and access to a reliable vehicle.

Required Certifications and Compliance:

  • NDIS Worker Screening Check (or willingness to obtain).
  • Completion of NDIS Quality and Safeguards Worker Orientation Training Module (or willingness to complete).
  • Working with Children Check (if applicable).
  • Current NSW Driver's License and a vehicle with third-party property insurance.
