This role is based in Port Pirie or Port Augusta, supporting clients across the surrounding region. We are looking for a candidate available to work full-time hours (negotiable for the right person).
The Role
We are seeking a dedicated and experienced Client Services Coordinator (Support at Home and NDIS) to join our growing team.
This pivotal role involves coordinating and managing Home Care Package and NDIS client services, ensuring that all clients receive high-quality, person-centred support aligned with their goals and care needs.
You will supervise and mentor a team of Support Workers, manage client care plans, and maintain close communication with clients, their families, and allied health professionals.
We are seeking someone with a clinical or social work background who is passionate about community-based care and leading teams to deliver exceptional outcomes.
Other key duties include:
- Coordinate and oversee the delivery of services for clients under Home Care Packages and NDIS.
- Accurately identify and respond to changes in clients' needs using assessment tools such as the AWACCS instrument and other clinical processes.
- Escalate identified client patterns or issues to CSI management for review and program development.
- Lead, mentor, and support a team of Support Workers to ensure services meet client goals and organisational standards.
- Ensure WHS compliance across client homes and oversee incident reporting and follow-up.
- Manage and respond to feedback, incidents, and changes in client circumstances promptly and professionally.
- Assist with the development and implementation of recruitment strategies aligned with client needs.
- Prepare reports, assessments, and documentation relevant to client care and service delivery.
Key Experience and Requirements Required For The Role:
- Diploma in Nursing or equivalent qualification (highly desirable).
- Strong understanding of My Aged Care Support at Home Packages and the NDIS framework.
- Proven experience in client coordination, case management, or clinical supervision within the community sector.
- Excellent leadership, communication, and problem-solving skills.
- Demonstrated ability to work autonomously and manage competing priorities.
- Strong understanding of compliance, safeguarding, and WHS requirements.
- Current NDIS Worker Clearance, DHS Working with Children Check, Police Check, and Driver's Licence.
