This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks' notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.
Are you a caring health professional ready to make a real difference for older adults with mental health needs? Central Coast Local Health District is looking for a Health Clinician Level 3 to join our Older Persons Mental Health (OPMH) Team at Wyong. This is an exciting opportunity for a newly qualified Level 3 clinician or an experienced Level 2 clinician specialising in Older Persons/Mental Health or Aged Care.

About The Role
As an OPMH Care Coordinator, you will work with people over 65 living with moderate to severe mental illness, including those in residential aged care. You'll provide care coordination, case management, recovery-focused interventions, and support for families and carers. Using your advanced clinical reasoning and expertise, you'll conduct mental health assessments, deliver evidence-based treatments, and make referrals in line with best practice standards.

You'll work both independently and collaboratively within a multidisciplinary team, helping older adults and their families navigate complex mental health needs and improve their quality of life.

Position Description

  • Hold a relevant tertiary qualification (Psychology, Clinical Psychology, Occupational Therapy, Nursing, Social Work, Counselling, or Community Welfare) with the required registration or membership and at least three years post-graduate experience in mental health, or a Master's in Clinical Psychology with endorsement.
  • Have experience in care coordination for older persons with complex needs.
  • Understand the mental health needs of older adults and their families and provide sensitive, person-centred care.
  • Can work independently and as part of a multidisciplinary team.
