About the role

We're currently seeking an Experienced Care Coordinator with aged care experience to join our team in Hope Island. This role is ideal for a Registered Nurse or Enrolled Nurse who is passionate about community care and ready to make a meaningful impact.

We offer flexible permanent part-time or full-time positions to suit your work/life balance.

Key Responsibilities

As a Care Partner, you will:

  • Assess Client Needs: Conduct initial and ongoing assessments to identify physical, emotional, and social needs, risks, and goals.
  • Care Planning: Develop, review, and update person-centred care plans aligned with Support at Home funding.
  • Service Coordination: Organise in-home supports and liaise with external providers to ensure timely, high-quality care.
  • Advocacy & Education: Help clients understand their funding, choices, and rights, and advocate for access to appropriate services.
  • Budget Oversight: Monitor individual budgets and guide clients on effective use of funds.
  • Relationship Building: Foster strong connections with clients, carers, and providers to support smooth transitions and ongoing care.
  • Compliance & Safety: Maintain accurate documentation, uphold aged care standards, and proactively manage risks.

Essential Criteria:

  • Strong communication and advanced problem-solving skills
  • Passion for high-quality, client-focused care
  • Bachelor of Nursing or Diploma of Enrolled Nursing
  • Current First Aid & CPR certification
  • Valid Australian National Police Check
  • Experience in community-based care coordination
  • Valid Driver's Licence and reliable vehicle
  • High level of computer literacy and tech confidence
  • Valid Australian work rights (please note: sponsorship is not available)
