Are you a seasoned Registered Nurse who thrives on variety, autonomy, and meaningful client connections?

As a Senior Clinical Coordinator, you'll play a key role in supporting older Australians to live independently and with dignity in their own homes. Your primary role will be client triage, intake and coordinating of clinical care for our Transition Care Program (TCP) clients. You'll also work alongside our office based clinical coordinator, in providing tele-based point of care support for our well-established nursing team

This is a role for nurses who want to step away from the hospital floor but still make a powerful impact every day.

What You'll Be Doing

  • Coordinate client intake and triage, working closely with hospital discharge teams and health networks
  • Partner with clients and families to co-design care plans that reflect their goals, preferences, and clinical needs.
  • Support reablement and restorative care that helps people regain confidence and independence at home.
  • Support our road-based community nursing team by providing tele-based point of care support by guiding nursing practice for our Home Care Package (HCP) and Support at Home (SAH) clients.
  • Step in to provide hands-on nursing care during staff leave or peak periods.
  • Participate in the on-call roster, ensuring continuity of care after hours.
  • Monitor progress, update care plans, and ensure services align with funding and best practice.

What You'll Bring

  • 8+ years' experience as a Registered Nurse, with a strong background in community or aged care.
  • Confidence in clinical assessment, care planning, and coordination.
  • Experience with reablement, restorative care, and end-of-life pathways.
  • A calm, compassionate approach and the ability to navigate complex client needs.
  • Strong documentation, time management, and communication skills.
  • A love for working independently while being part of a supportive team.

What You'll Need

  • Bachelor of Nursing + AHPRA Registration
  • Current First Aid Certificate
  • SA Driver's Licence + Reliable Vehicle + Comprehensive Insurance
  • DHS NDIS Worker Screening
