Your new role:

As a Nurse Navigator - Disability, you will provide advanced clinical leadership and coordinate complex care for patients across the Logan Hospital catchment. This role focuses on improving patient outcomes, streamlining services, and creating a more integrated healthcare journey for people with disability and their families.

Provide advanced nursing leadership and expertise in disability care.

Coordinate patient-centred care across primary, secondary, and tertiary providers.

Engage patients and families in self-management, advocacy, and informed decision-making.

Drive quality improvement, research, and system innovation to improve outcomes.

About you:

We are seeking a highly skilled and compassionate nurse who thrives in complex environments and is committed to advocating for people with disability.

Current AHPRA registration as a Registered Nurse with extensive clinical experience.

Advanced knowledge of disability care, including NDIS, guardianship, and restrictive practices.

Proven leadership, problem-solving, and change management skills.

Strong communication and ability to build collaborative partnerships across healthcare teams.
