Just Better Care Hills to Hornsby are looking for a Clinical Care Partner to join our caring and dynamic office team.

About the role:
Our Clinical Care Partner will perform an integral role in ensuring individualised services are developed and delivered to customers. You will be responsible for the development, maintenance and review of individualised support plans in consultation with the participant, their family or guardians and the key health professionals. You will also be responsible for training of our support workers, especially for the individualised services for our customers.
Responsibilities will include:
- On boarding new customers with comprehensive assessments, budgets and care planning
- Clinical oversight which ensures the delivery of quality care
- Clinical care eg wound care
- Working with multidisciplinary teams and other stakeholders
- Industry networking
- Care reviews; ensuring care plans reflect the ongoing needs of the individual
- Documentation in compliance with all guidelines and regulatory standards
- Provision of education and support to staff, clients and/or families
- Contributing to the Continuous Improvement of the Just Better Care Hills to Hornsby business
About you:
- Registered Nurse with AHPRA registration
- Certificate IV in Workplace Training and Assessment (or willingness to obtain)
- Previous experience in community care, demonstrating person centred care
- Highly efficient administrator, with business management skills and proven time management skills
Knowledge of Support at Home, NDIS and relevant guidelines
Ability to work flexibly to respond to changing customer needs.
Experience working in a multi-disciplinary team.
Ability to develop and maintain links and network with specialist services.
Experience maintaining computerised customer information systems.
High level of written and verbal communication skills.
Demonstrated ability to contribute positively to the team.
Current NDIS Worker Screening (or willingness to obtain)
Current First Aid & CPR (or willingness to obtain)
Current Drivers Licence and reliable vehicle.

Job Type: Part Time Job Share roll
Schedule:
- 8 hour shift
- Monday to Wednesday or Wednesday to Friday
- Cover for Job Share Co-Worker Holidays
- On call Bi-weekly
Experience:
- Registered Nurse: 2 years (Required)
- Demonstrated understanding of the Support at Home Program
Licence/Certification:
- Drivers Licence (Required)
