Commonwealth Home Support Program (CHSP) Coordinator

About the Role
As the CHSP Coordinator, you'll play a pivotal role in designing and delivering service packages that are commercially minded, creative, and client focused. You'll think outside the box to tailor solutions that are affordable, effective, and high-quality, ensuring our clients receive the best possible support.

You'll also bring strong administrative capabilities to the role, confidently managing billing, co-contributions, and reporting in line with government guidelines. Your key responsibilities will include:

  • Building and maintaining trusted relationships with clients, RAS assessors, referral agencies, and allied health professionals.

  • Creating and coordinating service packages that meet client needs while aligning with CHSP targets and funding requirements.

  • Monitoring service outputs and ensuring compliance with CHSP guidelines.

  • Managing bookings, billing, and reporting with accuracy and efficiency.

  • Supporting clients with complex needs and escalating feedback appropriately.

  • Promoting Lutheran Services as a provider of choice within the local community.

About You

  • Experience in aged care, home care, or case management.

  • Strong administrative and customer service skills.

  • Excellent communication and stakeholder engagement abilities.

  • Knowledge of CHSP guidelines and aged care legislation, or a willingness to learn.

  • The ability to work autonomously and manage competing priorities.

  • A current driver's licence and a valid police check.
