Chemist2U is Australia's leading digital pharmacy service, dedicated to making medication easy for patients. We partner with GPs to provide Home Medicines Reviews (HMRs), ensuring patients receive personalised medication management in the comfort of their homes.

Position Overview

We're looking for HMR-accredited pharmacists to join our expanding team. In this role, you'll make a real impact on patient care by conducting comprehensive medication reviews and working closely with GPs. Opportunities are available across
QLD, NSW, WA, SA, and TAS
.

Key Responsibilities

  • Patient Engagement:
    Book home visit appointments with patients referred for HMRs.
  • Pre-Screening:
    Complete pre-screening questionnaires to gather relevant patient information.
  • Medication Review:
    Conduct thorough HMRs, assessing the safety, effectiveness, and appropriateness of prescribed medications.
  • Reporting:
    Prepare detailed HMR reports for the referring GP, highlighting findings and recommendations.
  • Collaboration:
    Maintain strong communication with the Chemist2U support team to ensure seamless service delivery.

Qualifications & Requirements

  • Accredited with the Australian Association of Consultant Pharmacy (AACP) and HMR accredited.
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Professional liability and public indemnity insurance.
  • Previous experience conducting HMRs in a community pharmacy setting.
  • Strong written and verbal communication skills.
  • Ability to work independently while fostering collaborative relationships.
  • Full working rights in Australia.
