Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management and in home and community support services can be tailored to an individual's needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

The opportunity:
We are looking for a Peer Support Worker to join our supportive and professional multidisciplinary teams in on a fixed term contract working part time (0.8FTE).
Responsibilities include but are not limited to;
- Intentionally and safely share your lived experience of recovery.
- Provide a connection to care and social supports.
- Provide targeted follow up and assertive outreach to people during their recovery journey, bringing a lived experience/peer understanding to the relationship.
- Engage with individuals, community groups and clients who are risk of/have mental health issues.
- Support clients through intake and assessment processes and refer to other services as needed.
- Work with staff across varies programs to assertively engage with the target group, using a strengths-based, recovery oriented, hopeful and welcoming approach.
- Work as part of a multidisciplinary team within a supportive, inclusive, and diverse workplace.
- Provide a consumer perspective and contribute to planning, delivery, evaluation and promotion of our mental health services.
- Develop, maintain and enhance relationships with key stakeholders from a wide range of organisations.
Our ideal candidate:
- Lived experience of recovery from mental health issues.
- Experience in the provision of intentional peer support for people experiencing mental health issues.
- Experience working in mental health or a related field would be highly desirable.
- The ability to develop effective, client-centred therapeuticrelationships with clients/families/carers, by listening effectively,sharing information and understanding client and family needs.
- Able to demonstrate empathy, particularly in the development of the client-peer worker relationship.
- Effective motivational skills within a client centred framework.
- Ability to advocate appropriately for clients, the profession and healthAbility.
