LME Care, a leading disability support provider in Brisbane, is seeking enthusiastic and compassionate Community Support Worker to join our team. As a Community Support Worker you will play a crucial role in empowering individuals with disabilities to participate in community activities, develop social connections, and pursue their interests and hobbies. You will support clients in various settings, including recreational activities, educational programs, and vocational opportunities.

Responsibilities
  • Provide direct support to individuals with complex behaviors
  • Collaborate with clients, their families, and other stakeholders to create individualized support plans
  • Monitor client behavior and document any changes or incidents
  • Engage clients in meaningful activities that foster social connections and skill development
  • Ensure a safe environment for clients and promote positive behavior interventions
  • Promote independence and empower clients to make choices
  • Document client progress and report any concerns to the relevant team members
  • Participate in team meetings and training sessions to enhance skills and knowledge
  • Adhere to all relevant policies and procedures, including health and safety regulations
Requirements
  • Experience supporting individuals with complex behaviors and mental health issues in a community setting
  • Certificate III in Individual Disability Support required
  • Demonstrated understanding of Positive Behavior Support principles
  • MAYBO, MHFA, Crisis Intervention or Alcohol/Addiction training highly desired
  • Strong communication and interpersonal skills
  • Current first aid and CPR certification preferred
  • Valid driver's license and access to a reliable vehicle required
  • A current NDIS Workers Screening Card and a Working with Children Check (Blue Card)

Remuneration:

SCHADS Award - Social & Community Services

$64,000 - $74,000 per annum
