Description: The Chronic Conditions Care Co-ordinator role is to provide culturally enhanced clinical care and management to clients of Viney Morgan AMS, in particular those clients who suffer chronic health conditions.

Key Selection Criteria:

  • Demonstrated understanding and commitment to Victorian Aboriginal health, local Aboriginal culture, and the philosophy and practice of Aboriginal Community Control health service.
  • Demonstrated ability to work with Aboriginal organisations, communities and individuals in culturally appropriate ways
  • Demonstrated understanding of the diverse health and social needs of Aboriginal people
  • Well-developed interpersonal skills, including a friendly and professional approach and an ability to deal with a broad range of people at all levels.
  • Sound written verbal and communication skills
  • Demonstrated organisational and time management skills, including an ability to prioritise, manage multiple tasks, work to deadlines, manage interruptions and adapt to changing priorities
  • Demonstrated capacity to manage sensitive information, maintain confidentiality and remain impartial at all times.
  • Self-motivated and demonstrated ability of working independently with minimal supervision, and as an effective team member, promoting cooperation and commitment to achieve goals.
  • Demonstrated proficiency and experience using Microsoft applications (e.g. Microsoft Word, Excel, Outlook) combined with a high level of accuracy and attention to detail.
  • Previous experience in community support role and/or formal qualification relevant to role.

Duties include (please see position description for full details):

  • Provide culturally appropriate primary health care, health assessment, early intervention, case management, health promotion and health education within a team environment.
  • Managing patient care plans, scheduling appointments with specialists and allied health professionals, and coordinating referrals based on GP management plan.
  • Educating patients about their chronic conditions, promoting self-management strategies, and addressing any barriers to accessing healthcare.
  • Understanding and respecting the cultural needs of Aboriginal and Torres Strait Islander patients, ensuring culturally appropriate care delivery.
  • Working closely with GPs, Aboriginal Medical Services (AMS), other healthcare providers, and community organizations to ensure coordinated care.
  • Tracking patient progress, monitoring health outcomes, and generating reports for program evaluation.
  • Actively participate in evidence-based practice, work within the accepted nursing scope of practice and maintain nursing standards according to the ANCI competencies and competency standards for practice nurses.
  • Actively participate in the development, implementation and review of policies, protocols and systems as required.
  • Participate in accreditation processes and support ongoing quality assurance practice within the Medical Clinic.
  • Utilise available resources for identification of and intervention in potential health problems of individuals and their families.
  • Provide prompt and appropriate interventions to individuals and the community who have identified health problems to maintain or improve their health and wellbeing.
  • Collect and maintain accurate patient information and medical records using Communicare and other designated software programs. (Training provided)
  • Participate in the implementation of an effective referral and recall system responsive to client need.
