About The Role
The Care Coordinator/Senior Clinician physiotherapy, in addition to providing care coordination and discipline specific clinical services to complex care clients, provides clinical leadership within ALERT in both Physiotherapy specific and the care coordination role. The Physiotherapy Senior Clinician responsibilities involve the coordination and leadership of Physiotherapy staff, liaison with SVHM Physiotherapy department, and supporting professional development activities. This leadership role will include documentation reviews, improvement planning, peer group support and/or individual supervision and enabling of professional development opportunities. As a care coordinator, delivery of complex holistic health assessments and the planning for person centred client driven goals focusing on care co-ordination and improving self-management will be required. This involves direct and indirect clinical care, maintaining an active caseload and supporting clients to develop self-management skills in relation to their complex care needs and/or chronic disease.

Classifications for this position will range from VB7 - VC1 ($104, $118,981.20 per annum, pro rata for part time).

Your Contribution

  • Minimum 7 years experience in relevant acute/community setting
  • Excellent clinical skills and knowledge of best practice in relation to Physiotherapy specific interventions and care coordination of clients with chronic and complex needs
  • Experience in the knowledge and practice of client self-management models

  • Ability to engage with clients and assist them to identify their health and wellbeing needs and co-ordinates services and providers to achieve desired outcomes

  • Extensive discharge planning and case conference experience, health service liaison and networking
  • Experience with staffing responsibilities including recruitment, supervision, and identification of staff learning and development needs
  • Ability and willingness to take on committee work, consultative and teaching roles, including the ability to organise and chair effective meetings
  • High-level skills in Microsoft Office software
  • Experience in quality, evaluation and research activities
  • Hold a current Victorian driver's licence

About Healthcare For The Homeless
This position is based at St Vincent's Hospital Melbourne within the Emergency Department and providing outreach into the community. It involves working as part of an interdisciplinary team in a hybrid care coordinator and physiotherapist model.

This position is part of ALERT within the Healthcare for the Homeless program (HftH). HftH has a base of services within St Vincent's Hospital Melbourne and currently supports clients who are experiencing or are at risk of homelessness, and other psychosocial vulnerabilities. This service aims to integrate and coordinate existing services within St Vincent's Hospital and guide the development of new services to meet the need of the target client group.

Key target groups include patients with complex needs who are impacted by one or more of the following key factors: substance use, mental health issues, homelessness, disability, complex aged care issues, family violence and Aboriginal health.

Operationally the role has strong links with the Health Independence Program (HIP). HIP offers a comprehensive range of specialist clinics, services and rehabilitation programs, providing rapid response health management, short-medium term care coordination, specific allied health interventions and health education. HIP enables clients to be the decision makers of their care, and support clients through self –management to achieve their healthcare goals.

Hours of Duty:

  • Shifts can be hrs or ; 7 day week roster
  • Weekends and public holidays as rostered

POSITION PURPOSE

POSITION DUTIES
 Manages a case load of Complex Care clients

 Provides secondary consultation in regard to Physiotherapy and other areas of expertise across

ALERT and Complex Care Services (CCS)

 Provides leadership to Physiotherapy staff across ALERT and Complex Care teams and sites, including being the professional liaison with SVHM allied health department

 Ensures regular liaison, communication and consultation with the SVHM Physiotherapy department to support integration and maintenance of professional standards

 In conjunction with ALERT and CCS Managers leads the development, implementation and delivery of a high quality Physiotherapy service to clients of the Complex Care Service.

 Contributes to the planning and development of activities and services of Physiotherapy Service appropriate to clients.

 Ensures effective management of Physiotherapy aids and equipment to maximise patient independence and safety, and minimise organisational risk.

 Actively participates in the evaluation of the effectiveness of the Physiotherapy Service and/or identifies, supports or leads Physiotherapy specific quality improvement activities.

 Provides Physiotherapy and care coordination clinical expertise and leadership in clinical/team meetings.

 Provision of Physiotherapy specific and care coordination supervision

 Responsibilities for portfolios as negotiated with Manager

 Participates in the recruitment and orientation of staff as delegated by

Manager

 Physiotherapy Qualifications
8.1 ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS

 Current AHPRA registration

 Eligible for membership with Physiotherapy Association of Australia.

8.2 Other Essential Requirements
