About the Transition and Restorative Care Programs
The Transition and Restorative Care Programs sit within Aged Care Community Services as part of the Continuing Care Division at Austin Health. The programs are committed to providing high quality safe care for patients while helping them achieve their goals. The programs are provided in both residential and community settings.
About the Role:
Nurse Practitioners (NP) are supported by Austin Health to work in clinical specialties where there is evidence that the advanced clinical skills of a Nurse Practitioner would enhance patient outcomes within an efficient service model.
The Nurse Practitioner within the Transition Care Program (TCP) is responsible in conjunction with the Geriatricians in providing comprehensive medical assessments to clients admitted in the Residential or Community Programs.
The TCP Nurse Practitioner will work with the Multidisciplinary Team to ensure that best practice care is provided to clients on the TCP program, supporting avoidable hospital presentations and facilitating discharge planning.
Additionally, the position contributes to the department in other ways such as; education of staff, families and the community, research, practice and professional development, specialty and role promotion, quality improvement and clinical and professional leadership.
About you
You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You'll bring energy and drive to your work, and have a keen interest in developing your skillset.
You'll further have:
- Current AHPRA registration
- Nursing & Midwifery Board of Australia endorsement as a Nurse Practitioner
- Extensive and recent clinical experience in Aged Care
- Hold a valid Australian prescriber number
- Demonstrated commitment to ongoing personal and professional development, as evidenced in a Continuing Professional Development (CPD) record/Professional Practice Portfolio (PPP)
- Excellent interpersonal and communication skills that support patient care, team engagement and collaboration
- A flexible, innovative team-oriented approach to service delivery
- A positive approach to change and diversity.
