What You'll Be Doing
The Registered Nurse/Child and Family Health Nurse (CFHN) position is responsible for the provision of culturally safe, compassionate, quality, client centred care and work in collaboration with patients/families/carers and a multidisciplinary team to achieve identified goals and outcomes.
The role involves the provision of services to Aboriginal families within a Sustained Health Home visiting model. The position will work collaboratively with Aboriginal Health Workers as well as social work to provide assessment, care coordination and education to families and take into account clinical, psychosocial and cultural needs.
The program works with women with an Aboriginal baby and their families, from the antenatal period until the child is 5 years of age. This will include home visiting and practical support based on primary health care principles to promote a safe and secure environment for children. The aim is to better respond to and provide early intervention and prevention care interventions for families in need of additional support and those with complex needs.

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.
These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.
With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

- Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). Child and Family Health nursing qualifications or commitment to undertake and complete in first 12 months, with relevant experience.
